Blood tests for everything under the sun, including some very scary (and mostly rare) things, but the most likely cause is Rocky Mountain Spotted Fever.

The tilde part is we know what to do about that. Long course of antibiotics, then weeks or months for the blood to filter out of the tissue.

One negative is the ER should have been able to tell it was blood, not lymph. Fuckers.
